The Home Care Program allows patients to remain at home while recovering from illness or surgery. The primary care nurse or therapist works with the patient’s physician to design a recovery program to meet the specific needs of each patient. The patient’s wishes, comfort and well-being are given top priority. Home care patients are usually expected to regain as much independence as possible upon recovery.

Services Offered  

• Skilled Nursing
Registered nurses provide skilled nursing care based upon the plan of care established by your physician. Services are available seven days per week, twenty-four hours per day, 365 days per year.

• Physical and Occupational therapy
Registered physical therapists and licensed occupational therapists provide therapy and exercises as ordered by your physician to help regain mobility, strength and ability to manage activities of normal living.

• Speech/Language pathology
Certified and licensed speech/language therapists provide retraining of impaired speech, assistance with swallowing disorders and other specialized therapy pertaining to speech and language deficits.

• Medical social worker services
Licensed medical social workers provide supportive counseling to patients and families who are coping with illness, assistance to access community resources, and assistance with obtaining financial resources as needed.

• Nutritional guidance
Registered dieticians provide teaching and guidance regarding special diets, nutrition, meal planning/preparation and other diet-related issues.


• Homemaker, Home Health Aide services
Homemakers provide light housekeeping, laundry, grocery shopping and meal preparation. Certified home health aides provide assistance with bathing, dressing, prescribed exercises, feeding and other personal care as instructed by the registered nurse or therapist.


• Private Duty Aide Program
Home Health Aides provide private duty care for two hour shifts or more to provide personal care, bathing, meal preparation, light housekeeping and companionship.
